In May 1998, Malthouse was elected to Westminster City Council. He represented the St George's ward in central London. After some boundary changes, he was re-elected in 2002 for Warwick ward. He became the Chief Whip for the Conservative group. Later, he was appointed Chairman of the Social Services Committee. Two years later, he became the Deputy Leader of the Council. He was also in charge of finance for the council.
He left the Westminster City Council in May 2006. Malthouse questioned the 2001 population count for the City of Westminster. He believed it was too low. After two years, the population count was increased by 10%.
Malthouse was against the London congestion charge. This charge makes drivers pay to enter certain parts of London. He felt it should not be started in England's busiest city. He also thought London was already a very expensive place to live.

While Deputy Mayor, Malthouse shared concerns about the increasing number of foxes in London. He said people were worried about letting small children play outside. He later stated he would vote to change the law that bans fox hunting with dogs.

In March 2016, a charity called the MS Society in Andover asked Malthouse to step down as their patron. This was because he had voted to reduce certain benefits for people with disabilities.
