The stream begins, or its headwaters are, right next to Missouri Route 19. This spot is about three miles (about 4.8 kilometers) north of Salem. It's also about one mile (about 1.6 kilometers) south of a small community named Howes.

Nelson Branch was named after a local family who lived in the area. The Nelson family gave their name to this stream.
