When Adreian started at Michigan State University, he played in 34 games. He averaged about 2.5 points and 2.4 rebounds per game. During this time, doctors found out he had a condition that made his lungs work less well. This sometimes affected how long he could play without getting tired.

In his second year, Adreian became a key player for the Spartans. He led the team in blocked shots with 39. He also averaged 7 points and 4.2 rebounds per game. He was recognized for his good grades too, earning a spot on the Academic All-Big Ten team.

By his junior year, Adreian was one of the best free-throw shooters in the Big Ten conference. He made 84.8% of his free throws. He also blocked 46 shots, which was one of the best numbers in Michigan State history. He averaged 10.5 points and 7.6 rebounds per game. Adreian was named to the Second Team All-Big Ten, a big honor for college players.

Before his senior year, Adreian was considered one of the top college players in the nation. He was on watchlists for major awards like the Naismith and Wooden Awards. He missed a few games due to an ankle injury. Even with the injury, he was still named Second Team All-Big Ten at the end of the season.

Friendship with Lacey Holsworth

During the 2013–14 college basketball season, Adreian became very close friends with Lacey Holsworth. Lacey was an 8-year-old girl who was battling cancer. Their friendship began in 2011 when Adreian met her during a team visit to a hospital. They often texted and talked after that first meeting.

Lacey had a type of cancer called neuroblastoma. She was a huge fan of Adreian and the Spartans. She even joined Adreian at center court on Senior Night. She also helped him cut down the nets after the team won the Big Ten men's basketball tournament. Lacey, who was also known as "Princess Lacey," sadly passed away from her cancer on April 8, 2014. Their friendship touched many people across the country.
