He also completed special training called medical internships. These internships were at the University of Kentucky and Saint Elizabeth Hospital in Utica, New York.

Outside of his political role, Donald Douglas is a medical director and lead physician. He works at the Tony Delk IMAC Regeneration Center in Lexington, Kentucky. This means he helps lead the medical team and cares for patients there.

In November 2021, he was elected to the Kentucky Senate in a special election. A special election happens when a political position becomes empty before the usual election time. When he took office, he made history. He became the first African-American Republican state senator in Kentucky.
