The Pavlov First Saint Petersburg State Medical University started in September 1897. It was first called the Medical Institute for Women. This was a big step for women's education at the time. Money from Lydia Shanyavskaya, who worked for women's rights, helped start the institute. Marta Helena Nobel-Oleinikoff, who was Alfred Nobel's niece, also gave a large donation. She was a former student of the institute.

Over the years, the university's name changed several times. In 1918, it became the Women’s Medical Institute of St. Petersburg. Then, in 1924, it was renamed the First Medical Institute of Leningrad. In 1936, it was named after the Nobel Prize winner Ivan Pavlov.

In 1994, the institute became a full medical university. It was called Pavlov Saint Petersburg State Medical University. The word 'first' was added back to its name in 2013.
