The Wyaconda River begins in west-central Clark County. It forms where two smaller rivers, the North Wyaconda and the South Wyaconda, meet. This meeting point is about 3.5 miles east-northeast of the town of Wyaconda.

From its start, the river flows southeast. It enters Lewis County about 1.5 miles southwest of St. Patrick. The river then turns south, passing under Missouri Route 16 near Canton.
The Wyaconda River continues its journey southeast. It flows alongside US Route 61 for a while. Finally, it meanders into the Mississippi River just north of La Grange. The river joins the Mississippi at an elevation of 472 feet.
