Cold Water Creek starts near a town called China Grove, North Carolina. Its journey begins about half a mile northeast of the town.

From its starting point, the creek flows generally towards the southeast. Then, it turns and flows south. It continues this path until it meets Irish Buffalo Creek. This meeting point is about 3 miles northeast of a place known as Flows Store.
