Milam was first settled around 1828 and was originally known as Red Mound. It was later renamed in honor of Benjamin Rush Milam, a well-known person from the Texas Revolution. This happened in 1836 when Milam became the main town, or "municipal seat," for Sabine County.
However, as more people moved into the area, they wanted the county seat to be in a more central spot. So, in 1858, a new town called Hemphill, Texas was chosen for that role instead. During the Civil War, the Confederate Army's Quartermaster (the person in charge of supplies) for Sabine County worked from Milam.
