The Rh blood group system is a way to classify your blood. It's based on special proteins found on the surface of your red blood cells. Think of these proteins as tiny flags on your blood cells. After the ABO blood group system (like A, B, AB, or O blood types), the Rh system is the most important for blood transfusions.
There are over 50 different Rh proteins, called antigens. The most important one is the D antigen. If you have the D antigen, you are considered Rh positive (Rh+). If you don't have it, you are Rh negative (Rh-). For example, if your blood type is A+, it means you have the A antigen and the Rh(D) antigen. If you are A-, you have the A antigen but not the Rh(D) antigen.
Knowing your Rh type is very important for safe blood transfusions and for pregnant people, as it can affect the baby's health.

Understanding Rh Names
Scientists use different ways to name the Rh factors. Early on, two main systems were used: one by Ronald Fisher and R. R. Race, and another by a scientist named Wiener. Each system had its own ideas about how these blood factors were passed down through families.
Over time, with the help of DNA testing, we learned that both systems were partly right! We now know there are two main genes involved: the RHD gene, which creates the D antigen, and the RHCE gene, which creates other important Rh antigens like C, c, E, and e. This understanding helps doctors and scientists work with blood types more accurately.
What are Rh Antigens?
Rh antigens are special proteins found on the surface of your red blood cells. The most important one is the D antigen. Other key Rh antigens include C, c, E, and e. These antigens are made by two genes in your body: the RHD gene makes the D antigen, and the RHCE gene makes the C, c, E, and e antigens.
If you are Rh positive, it means you have the D antigen. If you are Rh negative, you do not have the D antigen. It's important to know that there isn't a "d" antigen; the lowercase "d" simply means the D antigen is missing.
Rh Types Around the World
The frequency of Rh-negative blood types varies in different parts of the world. For example, the Rh-negative blood type is more common in people of European descent. In some populations, like the Basques in Europe, a higher percentage of people are Rh-negative. In contrast, it is very rare in many parts of Asia and among Native Americans.
How Your Body Reacts to Rh Antigens
Your body's immune system can create special defenders called Rh antibodies. These antibodies are usually made if you are Rh-negative and are exposed to Rh-positive blood. This can happen during a blood transfusion or during pregnancy.
The D antigen is the strongest Rh antigen. If an Rh-negative person receives Rh-positive blood, their body is very likely to make anti-D antibodies. These antibodies are important because they can cause problems during future transfusions or pregnancies.
Rh Factor and Pregnancy
One of the most important reasons to know about Rh factor is its role in pregnancy. If a pregnant person is Rh-negative and the baby is Rh-positive, it can sometimes lead to a condition called Rh D Hemolytic Disease of the Newborn (also known as Rh disease).
What is Rh Disease?
Rh disease happens when the Rh-negative parent's immune system makes antibodies against the Rh-positive baby's red blood cells. This usually doesn't affect the first Rh-positive baby because the parent's body might not have made enough antibodies yet. However, in later pregnancies with Rh-positive babies, these antibodies can cross into the baby's bloodstream and attack the baby's red blood cells.
- Hemolytic means the breaking down of red blood cells.
- Erythroblastosis refers to the body making immature red blood cells to replace the ones being destroyed.
- Fetalis means it affects the fetus (the unborn baby).
Preventing Rh Disease
Thankfully, Rh disease is largely preventable today! Doctors can give Rh-negative pregnant people special injections of Rho(D) Immune Globulin (anti-D antibodies). These injections help prevent the parent's body from making its own antibodies against the baby's Rh-positive blood. These injections are usually given around 28 weeks of pregnancy and again after birth.
Without this prevention, a baby with Rh disease could face serious health problems, including severe anemia (not enough healthy red blood cells) and jaundice (yellow skin). In severe cases, it could be life-threatening for the baby.
Signs of Rh Disease
Doctors can look for signs of Rh disease in the baby using ultrasound scans. After birth, a baby with Rh disease might show:
- Pale skin due to anemia.
- Yellow skin or eyes (jaundice) caused by a substance called bilirubin.
- An enlarged liver or spleen.
- Swelling of the entire body (edema).
- Difficulty breathing.
Rh-like Conditions in Animals
While Rh disease as we know it mainly affects human babies, similar conditions can happen in some animals, like horses, mules, pigs, cats, cattle, and dogs. This is called Neonatal Isoerythrolysis (NI).
The main difference is how the antibodies reach the baby. In humans, antibodies can cross the placenta before birth. In these animals, the antibodies are passed to the newborn through the mother's first milk, called colostrum, right after birth. The baby animal is usually fine at birth but can develop problems after drinking this milk if their blood types are incompatible.
How Common are Rh Blood Types?
Globally, about 94% of people are Rh-positive, and about 6% are Rh-negative. However, these numbers can vary a lot depending on where you are in the world. For example, the Rh-negative blood type is more common in people of European descent compared to many other populations.
How Rh Factor is Inherited
Your Rh blood type is passed down to you from your parents through genes, just like eye color or hair color. The main gene for the D antigen is called RHD.
Dominant Trait
The D antigen is a dominant trait. This means if you inherit at least one copy of the gene for the D antigen, you will be Rh-positive. If both of your parents are Rh-negative, you will definitely be Rh-negative. If one or both parents are Rh-positive, you could be either Rh-positive or Rh-negative, depending on the specific genes they pass on.
The Genes Involved
The D antigen is made by the RHD gene. Other important Rh antigens (C, c, E, and e) are made by a different but similar gene called RHCE. Both of these genes are located on chromosome 1 in your body. There's also another gene, RHAG, on chromosome 6, which helps these Rh proteins work correctly on your red blood cells.
Testing for Rh Antigens
Doctors use special tests to find out your Rh blood type. These tests look for the Rh antigens on your red blood cells. Sometimes, more advanced tests, like red cell genotyping, can look at your genes to find out exactly which Rh antigens you have. This is very helpful for making sure blood transfusions are safe and for managing pregnancies.
What Do Rh Proteins Do?
Scientists believe that Rh proteins, like the RhD protein, act like tiny doors or channels on the surface of your red blood cells. They might help move gases like carbon dioxide (CO2) or ammonia (NH3) in and out of the cells.
Interestingly, some studies suggest that being Rh-positive, especially having one copy of the RhD gene (called heterozygosity), might offer some protection against certain effects of a common parasite called Toxoplasma.
Rh Proteins in Other Living Things
Rh-like proteins aren't just found in humans! They can also be found in other animals, and even in tiny worms, bacteria, and algae. In these organisms, these proteins also help move gases like CO2. This shows how important these proteins are for life across many different species.
Rare Rh Types
Sometimes, people can have very rare Rh blood types.
Weak D and Partial D
Some people have a "Weak D" phenotype, meaning they have the D antigen, but it's present in smaller amounts on their red blood cells. Others have "Partial D," where the D antigen is present, but its structure is slightly different.
These differences are important for blood banks. People with Weak D are usually considered Rh-positive when donating blood. However, when receiving blood, doctors might sometimes treat them as Rh-negative to be extra safe, especially for Partial D types, to prevent their body from making antibodies.
Golden Blood (Rhnull)
A very rare blood type is called Rhnull, sometimes known as "Golden Blood." People with this blood type have no Rh antigens at all on their red blood cells. This condition is extremely rare, with fewer than 50 individuals reported worldwide.
Because their red blood cells lack these important proteins, people with Rhnull blood can sometimes have health issues like hemolytic anemia, where their red blood cells break down too easily. This blood type is incredibly valuable for transfusions when someone with a very rare blood type needs blood.
The Story of Rh Factor Discovery
The term "Rh" originally came from "Rhesus factor." It was discovered in 1939 by scientists Karl Landsteiner and Alexander S. Wiener. They found an antigen in human blood that seemed similar to one they found in rhesus macaque monkeys.
Later, in 1940, other scientists, Philip Levine and Rufus Stetson, realized how important this discovery was. They linked the Rh factor to serious problems that could happen during blood transfusions and in newborns.
Even though scientists later found that the human Rh factor isn't exactly the same as the one in rhesus monkeys, the name "Rh" stuck because it was already widely used. Today, we use "Rh" as a special term to describe this important blood group system.
